Guidance Note for Patients Requesting Exemption Letters

Coronavirus Guidance

If you have travelled abroad in the last 14 days or been in close contact with someone with confirmed coronavirus please check the government’s website for the latest COVID-19 travel advice, you may need to call or go online to NHS111 for further advice before making or attending your appointment.

New Patient Registration

Firstly, please check with the receptionists that you live in our catchment area. See the practice area map here. They will then give each new patient two forms to fill in.

One form is sent off to the Health Authority requesting your full UK medical history from your previous surgery. The other form allows us to obtain a brief summary of this information while we wait for your full records.

As soon as these forms are filled in and the data entered on to our computer, you are able to see any of the medical staff in the practice.

Guide to GP Services

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
